Institutions that award graduate degrees are eligible to become recipients of ETS® Personal Potential Index (ETS® PPI) Evaluation Reports by having a "Designated Institution" ("DI") code assigned to them. Institutions that do not meet this requirement are, in general, not eligible to receive ETS PPI Evaluation Reports.
ETS reserves the right, at its sole discretion, to grant, deny, or revoke a DI code based on eligibility requirements or for any other reason, and to make exceptions to its policy, under special circumstances.
If your institution meets the requirements stated above, ETS will send you a letter or email with your DI code within approximately three (3) weeks after receiving your completed application.
If you are granted a DI code by ETS, applicants to your institution will be able to select your institution to receive an ETS PPI Evaluation Report.
Institutions that receive official GRE® score reports are automatically eligible to receive ETS PPI Evaluation Reports. Complete this form only if you do not wish to receive GRE score reports, but would like to receive ETS PPI Evaluation Reports. If you wish to receive both GRE score reports and ETS PPI Evaluation Reports, please complete the
ETS publishes a list of official ETS PPI Evaluation Report recipients on the PPI website at . This list is updated once per month.

The information on the ETS PPI Evaluation Report should be considered along with a graduate applicant's undergraduate grades, test scores, recommendation letters and other information that is submitted as part of the graduate application.
Recipients of ETS PPI Evaluation Reports may not, without the express, prior, written consent of ETS, copy, release, provide access to or otherwise disclose ETS PPI data to anyone except individuals within their particular organization having a need to know.
ETS reserves the right to monitor access and use of ETS PPI Evaluation Report data by all authorized recipients.
